About the Role:
APremium Healthcare Solution, LLC is looking for a compassionate and driven Patient Navigation Assistant to join their growing team in Conyers, GA. In this role, you'll be the guiding force that helps patients move seamlessly through their healthcare journey — making a real difference in people's lives every day. Responsibilities:
- Guide patients through the healthcare system, including scheduling appointments, referrals, and follow-up care coordination
- Serve as a primary point of contact for patients, addressing questions and concerns with empathy and professionalism
- Assist patients in understanding treatment plans, discharge instructions, and available healthcare resources
- Collaborate with clinical staff, physicians, and administrative teams to ensure continuity of care
- Maintain accurate and up-to-date patient records in the electronic health record (EHR) system
- Identify and help remove barriers to care, including insurance, transportation, and social determinants of health
- Monitor patient progress and proactively follow up to ensure adherence to care plans
Requirements:
- High school diploma or GED required; associate's or bachelor's degree in healthcare, social work, or related field preferred
- Prior experience in a healthcare, patient services, or care coordination setting strongly preferred
- Familiarity with medical terminology, EHR systems, and healthcare workflows
- Strong interpersonal and communication skills with a patient-first mindset
- Ability to multitask, prioritize, and problem-solve in a dynamic environment
- Knowledge of community health resources, insurance processes, and care management principles
- Bilingual candidates are a plus

Compensation: $30.00 - $45.00 per hour
